Terms and Conditions for the T.OT Fine Motor and Sensorimotor Camp

Participation in the T.OT camp is dependent on the following:

1. Child must be potty trained.

2. Masks are not required, but sick policy will be heavily enforced. If you have questions about this requirement, please message me via e-mail or DM me on Instagram at @ot.umma to further discuss.

3. Every child must attend the free 15 minute screener session prior to the start of the camp. A sign up sheet with time slots available will be sent out after registration.

4. At the screener, if it is determined that the T.OT Camp may not be a good fit for the child at this time, then a full refund minus $15 (the cost of the screener for children not continuing with the camp) will be issued in the method and the amount that the payment was received.

5. Child must show behavioral readiness to attend the program meaning the following:

The child must be able to follow directions.
The child must not display self-directed behaviors (e.g. crying, screaming, eloping, hitting, flopping, non-compliance, etc).
The child must be able to understand verbal directions given in English.

6. Refunds will be issued in the following circumstances:

Drop requests made more than 48 hours prior to the first day of camp. Less than 48 hours notice will be given a 75% refund of the amount paid in the method that the payment was made.
If at the screener it is determined that the T.OT Camp may not be an appropriate program at this time, a refund minus $15 (in the method of payment) or credit toward a future camp date will be issued.
NO REFUND will be given with respect to a child drop requests after the start of the camp. This includes monthly payment plans. Instead, possibility of a credit for a future camp session will be issued.
One make-up session may be available..

Covid-19 and Sick Policy

If a child is showing symptoms of Covid-19/sickness (e.g. fever, runny nose, cough, shortness of breath, fatigue, sneezing), the child will need to stay home. A negative covid test (e.g. home antigen test is sufficient) will be needed on the morning of the next camp session (text or e-mail the picture of the results) in order for the child to attend. If a child misses a session due to illness, I will try to accommodate a make-up session to the best of my ability. However, it is dependent on space availability in the other cohort.

Waiver & Liability Release

In consideration of being allowed to participate in any way in T.OT Fine Motor and Sensorimotor Camp at 161 Fashion Lane, #203, Tustin, CA 92780, I, the undersigned, acknowledge, understand, and agree that:

1. My child may risk bodily injury, including paralysis, dismemberment, disability, and death, and while particular rules of the skills, equipment, and personal discipline may reduce this risk, THIS RISK OF SERIOUS INJURY DOES EXIST, AS WELL AS THE RISK OF DAMAGE TO OR LOSS OF PROPERTY; and,

2. I knowingly and freely assume any and all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my child's participation; and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or my child's participation, or if I observe any concern in my child's readiness for participation, I will immediately bring such to the attention of the nearest official and remove my child from participation; and,

4. I, FOR MYSELF, and on behalf of my heirs, assigns, personal representatives, and the next of kin, HEREBY RELEASE, INDEMNIFY, HOLD, HARMLESS and PROMISE NOT TO SUE JENNELL CHANG (T.OT FINE MOTOR & SENSORIMOTOR CAMP) and BUILDING (Maude-Corona LLC and PacificWest Asset Management Corporation), their officers, officials, volunteers, employees, agents, and/or other participants, sponsors, advertisers, and, if applicable, the owners and lessors of premises used for the activity ("RELEASEES"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, and/or LOSS OR DAMAGE TO PERSON OR PROPERTY, WHETHER CAUSED BY THE NEGILIGENCE OF THE RELEASEES OR OTHERWISE, EXCEPT THAT WHICH IS THE RESULT OF GROSS NEGLIGENCE OR WANTON MISCONDUCT, to the fullest extent permitted by law.
